# Terms of Service

These Terms of Service ("Terms") govern your use of the website proathletept.com and services provided by PRO Athlete Physical Therapy LLC ("we," "us," or "our"). By using our website or services, you agree to these Terms.

## Services

PRO Athlete Physical Therapy provides sports physical therapy, arm care programs, and related services at our locations in Lakewood and Centennial, Colorado. All services are provided on a cash-pay basis. We do not bill insurance directly.

## Appointments and Cancellations

Appointments are scheduled through our website or by phone. We require advance notice for cancellations. Missed appointments without notice may result in a fee. Sessions must be used within 90 days of purchase.

## Payments

Payment is due at the time of service or according to the terms of your purchase. We accept credit cards and offer payment plans for qualifying purchases. All sales are final. Unused sessions are non-refundable.

## SMS and Communications

By providing your phone number and opting in to SMS, you consent to receive text messages from PRO Athlete Physical Therapy for appointment reminders, payment notifications, and clinic updates. Message frequency varies. Message and data rates may apply. You may opt out at any time by replying STOP to any message. Reply HELP for assistance. Consent to SMS is not required to purchase services.

## Website Use

You agree to use our website for lawful purposes only. All content on proathletept.com, including text, images, and logos, is owned by PRO Athlete Physical Therapy LLC and may not be reproduced without written permission.
